HANG CAN OCCUR WITH INTRA_PARALLELISM ENABLED WHEN INDEX GETS CREATED

| Problem description: |
An SQL query (including a stored procedure) can hang when used
with intra_parallelism enabled. A driving condition would be
an index that gets created (or recreated) in the same
transaction that spawned the query. |
